CLO - EFT Supports for Family Teachers

CLO provides Consulting Services along
with Health Services for all it's Extended
Family Teachers. The Extended Family Teacher Co-op is a self-administered
union of EFT teachers to share information and address common
concerns.
Consulting Services
As part of the EFT contract CLO provides Teaching Consultant Services to the EFT setting. The Teaching Consultant has several roles and responsibilities in their support to the EFT, and to the consumer living in the EFT home.
A primary responsibility of the Teaching Consultant is to work with the EFT to become a Certified Extended Family Teacher or to maintain existing Certification status. If an EFT is not yet Certified they can expect Consultation from thier Teaching Consultant twice a week in the form of training/observation & feedback. One of the two visits a week should occur the home setting. If you are Certified you should expect Consultation supports twice a month, one of which should occur in the home setting. Your Teaching Consultant will work with you to ensure that scheduling of these supports fits you family calendar as much as possible.
Additionally, they provide support with Life Arrangement through consultation with schedules, behavioral support planning, skill acquisition plans, treatment team planning and on-call support. Each month the Teaching Consultant assigned to your home completes a status report of the supports and services provided to your EFT home, which are designated by the level of service that your consumer requires.
The level of Consulting Services provided is specified solely from the needs of the consumer. These levels are:
- Level 1
- Presence of skill acquisition teaching plans
- No formal behavior support plans
- Does not attend Treatment Team
- Level 2
- Presence of skill acquisition teaching plans
- Presence of behavior support plan(s)
- Utilizes Psychotropic Medications
- Receives Treatment Team Support
- Level 3
- Presence of skill acquisition teaching plans
- Presence of behavior support plan(s)
- Utilizes Psychotropic Medications
- Receives Treatment Team Support (level 3 Tx. Tm.)
- Has a high frequency of intensity of maladaptive behaviors
- Requires or has required within the last year 3rd party Emergency Behavior Support
Note:
A consumer may move from Level 2 nursing support to Level
1 support under the following conditions:
- 6 months of living in the EFT program
- The EFT is certified
- The nursing staff (case nurse and Dir. Of Nursing) agree
and recommend the change
- The guardian approves the change.
- The EFT Development Team approves the change.

Health Services
As part of each EFT contract CLO provides health care oversight to the consumer you provide services to and health care service support for you, the EFT through CLO's Wellness Clinic.
The amount of service and support is dependent upon the health care needs of the person served. Each consumer starts out as a Level 2 unless the team (including the nurse), CLO's EFT contact and guardian feel that the consumers health needs are such that require additional oversight and support - in this case they would start as a Level 3.
Each month the health care professional assigned to your home completes a status report of the health care services provided to your EFT home, which are designated by the level of service that your consumer requires.
