Quality Assurance & The EFT Program

One of the foundations of all of CLO’s residential programming is that of ensuring that each of CLO’s Outcomes are present in the lives of those we serve and those that support them.  Within the EFT program we go about this in several ways:

1) Minimum Contractual Outcomes

The person served, their family/guardian, the Extended Family Teacher, and CLO work together to determine Minimum Contractual Outcomes (MCOs’).  These MCOs’ include both general and person-specific outcomes consistent with CLOs’ definition of Quality of Life for people with and without disabilities.  Each of CLOs’ outcome areas are defined for the person served within the MCO.  These outcome areas are evaluated quarterly and monitored through visits to the home.

2) Teacher Certification

All Extended Family Teachers are required by contract to become certified, which requires working with a CLO consultant to complete a curriculum that includes training, observation and feedback

3) Regular Visits

Visits both announced and unannounced, are made to the home by the Teaching Consultant, the Targeted Case Manager and CLO’s EFT Representative.

4) Quarterly Visits

Vists are conducted with CLO's EFT representative on a quarterly basis to review feedback from the EFTs’ support services (consulting, nursing, finance, targeted case management) as well as to review contractual requirements.

5) Reciprocal Satisfaction Evaluations

Feedback is solicited 3 weeks prior to a quarterly visit from the EFT supports (Consulting, Nursing, Finance, and TCM) as to the satisfaction of how services are provided within the EFT home.  Reciprocally, the EFT is asked to rate their satisfaction of these same supports during their quarterly visit.
